Online Consumer Complaint Procedure

  1. Download and fill up the Consumer Complaints form.
  2. Download and fill up the Verification and Certification of Non- Forum Shopping.
  3. Have the Verification and Certification of Non-Forum Shopping notarized.
  4. Submit both forms to the ERC Consumer Affairs Service, 12/F Pacific Center Building, Ortigas Center, Pasig City, 1600.
