Instructions in Requesting for an Acceptance Test

1. Download the following forms:
   a. Request for Acceptance Test Form
   b. Test Report
   
2. Accomplish the form by typing and enclosing all items listed therein, except for the portions reserved for accomplishment by the ERC staff. If a question is not applicable, type N/A in the space.
   
3. Double check the accomplished forms for completeness and accuracy, then send it to the following:
