5. Photocopy of Eligibility Certificate (e.g. CSC Eligibility and/or
Report of Rating of Board/Bar Exam)

Indicate your list of trainings, training hours, and institutions which conducted the training course you attended in your Personal Data Sheet. Otherwise, it will not be counted.
